This record form is used to monitor the development of a child’s narrative skills. It provides a more detailed record of the child’s narrative skills than is included in the Individual session record sheet to enable you to set appropriate targets to help you support the child in improving their story-telling. You should complete the narrative assessment form at least in Weeks 1, 10 and 20 to enable you to look back at progress made over the programme, but you should also re-complete it whenever the child meets the targets you have set for them in the current narrative assessment.
